We will also need the following information:

1. Medical Records: If you have been seen or treated by another physician, please be sure to bring your medical records, office notes, operative reports and if you have had a previous implant surgery, a copy of the Operative “sticky” labels.
2. X-rays
3. Insurance Cards
4. Referral: If you have an insurance carrier that requires a referral number, please obtain the referral prior to your appointment on all visits to our office.
   
   Plain Medicare does not require a referral.
